Individual and Family plans available. Packages can include add ons, and can be purchased monthly or annually. Once you’ve selected a package, you’ll be directed to payment processing.
Complete our registration and intake forms.
Once you’ve completed payment, you will be asked to complete the registration and intake forms. These forms provide us with consent to contact healthcare providers on your behalf for the purposes of storing your electronic records.
*please be sure to have your healthcare provider information available to get you started. However, additional providers can be added later.
These forms will be sent over to your healthcare provider by ULM and will follow up with your provider based on the anticipated time for electronic retrieval. In addition, you will receive notification when new records have been received in your secure portal.
Gain access to your portal.
Within 24 hours, you will receive access information to our secure self serve portal that can be accessed from any web accessible device. Here you can upload your own records and those that have been requested on your behalf from your healthcare provider.
